The Partnership for a Healthy Community (PFHC) is a community-driven partnership of public and private partners working together to address priority health issues in Peoria, Tazewell and Woodford Counties in Illinois. The Peoria City County Health Department (PCCHD) is accepting pre-proposals for the implementation of the 2026-2028 Community Health Improvement Plan (CHIP) strategies for Year 2026-2027. The 2026 CHIP Implementation grant is established by the PCCHD Board of Health to provide funding for CHIP plan projects and services primarily in Peoria County related to the CHIP plan health priorities for implementation. The three CHIP Plan health priorities include:
Preference will be given to projects and programs implemented by community partner members in the Partnership for a Healthy Community (PFHC) Action Teams:
For YR 2026-2027, funding will be awarded to (3) projects in the Tri-County area that have at least 50% of program/project implementation in Peoria County. The total funding award is $20,000 per grant. The CHIP Implementation grant funds will be disbursed on a reimbursement basis.
